The third number is the magnetic quantum number and tells you the configuration of the orbit given a specific orbital angular momentum.

The fourth number is the spin of the electron (up or down).

Those two electrons are both in the 4th energy level, with an orbital quantum number of 3 (f shell), but with different configurations (the projection of their orbital angular momentum to a specified axis is different). They are both spin up.
