Like the three other states, plasma consists of atoms. It is like a gas because the atoms are far enough apart to move freely in all directions. Unlike a gas, the particles are high enough in energy to freely exchange electrons as well. This makes plasma like an electrical conductor, which many solids and some liquids are. (plasma is what causes fluorescent lights to work).

To create a plasma, a gas has to acquire energy, either in the form of heat or, as in the case of fluorescent lighting, being exposed to high voltage. (the average tube runs around 10,000 volts, I think). In a fire, the plasma has acquired heat energy. 

Physical changes are the changes that change the physical traits of the substance, without making any change in their internal structure. On the other hand, a chemical change is one that affects the internal structure of the substance, so as to form a new substance.
