There are many responsibilities patients have related to communication to you as a health care professional. I'll state the most important ones:
-providing complete and accurate information, including cell phone numbers, address, insurance carriers and employer, date of birth, social security number, etc. but only when its required.
-To the best of your knowledge, you have to provide complete and accurate information about your medical and health history, including you present condition, past illnesses, hospital stays or visits, medicines, vitamins, allergies, etc.
-Make sure that the hospital or doctors office has a copy of your Advance Directives.
