Dr. Kim cannot simply diagnose right there and then that the cause of Joe being intensely annoyed by people on their cell phones while driving is cause by his cell phone irritation. This is because an instinct cannot explain complex human behaviors accurately.  A medical or psychological diagnosis entails a lot of comprehensive test before a conclusive cause can be obtained.
